Regeneration. The staldra regains 10 heart points at the start of its turn if it has at least 1 heart point.

Undying. The staldra can't be reduced to 0 heart points by taking 5 or less damage at once. At least 6 damage must be dealt at once, otherwise the staldra always retains at least 1 heart point.

Actions

Multiattack. The staldra makes three attacks.

Bite. Melee Ranged Attack: +5 to hit, reach 5 ft., one target. Hit: 7 (1d8 + 3) necrotic damage.

Necrotic Ray. Ranged Spell Attack: +0 to hit, range 60 ft., one target. Hit: 4 (1d8) necrotic damage.

“This is a snake monster from before the dawn of time. Three bone-plated heads and a thirst to cause pain keep it writhing even beyond death. My analysis shows that to defeat this cursed snake, all three of its heads must be simultaneously destroyed.”
— Fi the sword spirit, in Skyward Sword

These undying three-headed serpents have existed before the dawn of recorded history. While most undead can persist for centuries, few if any possess the sheer tenacity of a staldra. So long as one of their heads remains intact, a staldra can instantly recover even from seemingly lethal blows.
An Undying Relic. If this undead monster was created from a living creature, none remain who remember what animal that could be—perhaps some primordial form of gleeok. The magic which gave them life has long since been forgotten. Staldra are cursed to wander whatever long-abanded ruins or deserts have yet to put them out of their misey. Forever shouldering the agonizing burden of eternal decay, perhaps it is no surprise a staldra lash out at seemingly anything and anyone it comes across.
Undead Nature. The staldra does not require air, food, water, or sleep.
     Environment: Desert, Forest, Ruins
