This template should be placed at the bottom of any page regarding a subject that has been altered from its original source material. E.g., a creature's skill proficiencies have been altered to better suit it, or a magic item's properties have been minutely changed.
In addition, a summary of the change and the reason for it should be placed on the subject's talk page.
