Amphibious. The octo can breathe air and water.

Multi-Action. The octo can take 2 actions on each of its turns.

Siege Monster. The octo deals double damage to objects and structures.

Weak Point. Any target strike or sneakstrike which hits one of the octo's many eyes deals double damage.

Actions

Tentacle. Melee Weapon Attack: +11 to hit, reach 20 feet, one target. Hit: 29 (4d10 + 7) bludgeoning damage. If the target is a creature, it becomes grappled by the octo (escape DC 19).

Spit Ink. Ranged Weapon Attack: +4 to hit, range 150/600 feet, one target. Hit: 13 (2d12) acid damage, the target becomes soaked, and if the target is a creature it becomes blinded until it uses an action to clear the ink from its eyes.

Spit Rock. Ranged Weapon Attack: +4 to hit, range 150/600 feet, one target. Hit: 22 (4d10) bludgeoning damage.

Swallow. One Large or smaller creature grappled by the octo must succeed on a DC 19 Dexterity saving throw or become swallowed. A swallowed target is blinded and restrained, it has total cover against attacks and other effects outside the octo, and it takes 13 (2d12) acid damage at the start of each of the colssocto's turns. If the octo dies, a swallowed creature is no longer restrained by it and can escape from the corpse using 5 feet of movement, exiting prone.

Spit Up. The octo spits any number of targets it has swallowed a distance of up to 600 feet. The spit targets each take either 22 (4d10) bludgeoning damage, or damage appropriate to the distance fallen, whichever is greater. The creature lands prone unless it avoided this damage. As part of this action, the octo can treat the swallowed creature as the projectile used in its spit rock attack—a hit target takes damage from spit rock as normal.

Bonus Actions

Whirlpool. If the octo surfaces in a lake or ocean, it creates a whirlpool on the water's surface out to a radius of 60 feet. This water becomes difficult terrain. Any creature or ship which starts its turn in this water must succeed on a DC 15 Strength saving throw or be pulled 10 feet closer to the octo, and have its speed reduced to 0 until the start of its next turn. The whirlpool persists until the octo moves or becomes incapacitated.

Boss Mode

While the colossocto has a Challenge rating higher than any other monster in combat, its heart points are doubled, and it has advantage on initiative checks. It can also take 3 boss reactions each round, replacing its normal 1 reaction per round. The boss colossocto can use a boss reaction to Escape, Maneuver, Search, use any of its normal reactions, or do any of the following:

Boss Resistance (3/Day). When the octo would fail a saving throw, it can choose to lose 30 heart points and succeed on the save instead.

Cleave (Costs 2 Reactions). Immediately after the octo hits a creature with a melee attack, it can move a distance up to half its speed and make another melee attack against a different target. If its tentacle hits a creature with this reaction, the octo can't grapple the target.

“There are said to be six of these legendary giant squid in different reaches of the Great Sea. When ships pass by, they create tremendous currents to try to sink them.

The only way to escape is to shoot all of their weak spots - their eyes - with projectiles before being sucked in.”

— Figurine description, The Wind Waker

This monstrosity may only exist in legends told by sailors and pirates. Some say it's so massive that the mere act of it breaching water can sink a ship. None have lived to see it deliberately attack a vessel, but surely if it did there would be no survivors. If it exists, surely only the depths of the Great Sea could hope to house it.
