Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 9 (2d6 + 2) piercing damage, and if the target is a creature it must succeed on a DC 12 Strength saving throw save or be knocked prone.

Tail Slam. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 7 (2d4 + 2) bludgeoning damage, and if the target is a Medium or smaller creature it is pushed 5 feet directly away from the moldorm.

Encyclopedia

This skittering centipede-like monster is a line of boulder-sized spherical body segments led by a pair of bulbous googly eyes and imposing mandibles. A moldorm scurries erratically with wild and unpredictable movements only to throw its weight unpredictably into its opponent, often sending them flying. Moldorms are prevalent in hills and decaying dungeons, where their slamming tackles can send foes hurling over cliffs or through decayed floors.
